SJ CARES
San Joaquin Community Assessment, Response, and Engagement Services
A program designed to address homelessness by collaborating with local agencies, law enforcement, and community organizations to consolidate outreach initiatives throughout San Joaquin County. Our team works diligently to identify, assess, and connect individuals experiencing homelessness with the necessary supportive services available within the County.
We are committed to providing comprehensive wrap-around services tailored to meet the unique needs of each individual, resulting in a reduction of individuals experiencing homelessness. The goal of SJ CARES is to build relationships, gain trust, and encourage individuals to accept the services offered.
Our Team
SJ CARES is comprised of five probation officers, two mental health specialists, and one protective services social worker. They are divided into two teams: one team focuses on daily outreach and engagement, while the other team provides intensive case management services.
The outreach team works on building trusting relationships with individuals, creating personalized intervention and treatment plans, and connecting people to the right services and community support. They ensure that once someone accepts services, they are fully engaged and supported every step of the way.
The second team offers intensive case management services to homeless clients who may not qualify for traditional case management services. This team works together to create clear plans for each client, define everyone’s role in the plan, and agrees on how the client will receive services based on their unique needs.
Overall, SJ CARES is dedicated to helping individuals in need by providing personalized support and ensuring that no one falls through the cracks.
